Membership Application Procedure:
1. Download attached MUNREB APPLICATION FORM: MUNREB APPLICATION FORM No need to Print.
Fill up ONLINE or Using Adobe Acrobat Reader, SAVE AS PDF. Please check that all entries are saved before sending. OR FILL UP MANUALLY AND SCAN (PDF FORMAT)
2. Download attached PAREB APPLICATION FORM & PAREB ID FORM: PAREB APPLICATION FORM and PAREB ID FORM (please use these links for the forms) FILL UP MANUALLY AND SCAN (PDF FORMAT)
3. Email the accomplished MUNREB & PAREB Application forms and PAREB ID FORM (in pdf format) together with the following:
a. PRC License Certificate,
b. PRC License ID
c. Recent 2x2 ID Picture
Email Subject: (Your Fullname) - Application
Email to munreb.membership@gmail.com
4. Email Deposit Slip for Application Fee of Php 6,000 (BPI Checking Account No. 0331110078 Account Name: MUNREB). Email to munreb.membership@gmail.com
Breakdown: Php 1,000 MUNREB Application Fee / Php 1,000 PAREB Application and Annual Fee / *Php 4,000 MUNREB Annual Fee
*Munreb Annual Fee will be refunded should there be a problem with the membership application.
Please ensure before making a deposit that you have already emailed ALL the requirements above otherwise your application will NOT be processed.
5. Membership committee will notify applicant via email re status of application. Circularization of MUNREB membership (10 days), PAREB (45 days)
